Scheduling a Regular Office Appointment Our office in Raleigh, NC is open Monday through Thursday from 8:00 a.m. to 5:00 p.m.; closed for lunch from 12:00 p.m. to 01:00 p.m.
The urology specialists at Landmark help serve the needs of patients in cities across Wake County, to cities in Orange County, Durham County, Johnston County, Harnett County, Franklin County and Chatham County. These cities include Wendell, Wake Forest, Holly Springs, Apex, Garner, Knightdale, Zebulon, Rolesville, Morrisville, Cary, Fuquay-Varina, Benson, Dunn, Erwin, Four Oaks, Chapel Hill, Durham, Angier, Coats, Lillington, Youngsville, Louisburg and Pittsboro. Emergency calls & Urgent care In the event of a urological emergency, call 911 or proceed directly to the nearest emergency room. Alternatively, if the situation is not life-threatening, you may contact our office for further instructions. If the office is closed, please call our answering service at 919-872-4468. The answering service will contact the physician that is providing emergency call coverage. Initial Appointment If the registration forms are filled out in advance, we ask you to arrive fifteen minutes before your appointment time, otherwise please arrive thirty minutes before your appointment to allow sufficient time to register and complete the necessary paperwork. Remember to bring any X-rays, MRIs or other diagnostic information relevant to your problem. Tips to help you make the most of your office visit Thank you for entrusting your care with our office. If you have not already done so, please take a few moments to plan your visit before seeing one of our practitioners. Ask yourself, why am I here today? If there are several reasons, discuss the issue that is most important to you first. New patients: If you are a new patient to our practice, welcome. Please bring any pertinent lab and x-ray reports with you to your appointment. If you have had any recent x-ray studies of your urinary tract, please contact the radiology office where your study was performed and ask them to make a CD of your study and bring the CD with you to your appointment. If you do not have a copy of your lab or x-ray results, please contact your primary care practitioner or referring practitioner’s office staff and ask them to fax the relevant reports and office notes to our office prior to your visit. [Top] Established patients: If you are an established patient, we appreciate the opportunity to serve you. If you are monitored with periodic lab or x-ray studies, we will provide you with a requisition form for your tests. Please have the testing performed at least one week prior to your visit so that the result will be back in time for your appointment. At each office visit, you will be given a requisition form for tests to be performed before your next office appointment. If a requisition form was not given, or if the form is misplaced, please notify our office prior to your next visit. In this situation, we offer three options: We will leave a lab requisition form at the front desk that you can pick up at your convenience (please notify our staff so we can have the form ready for you); you can send a self-addressed stamped envelope and we will mail a lab requisition form (saves time and gas money); or you can wait until your next visit and we will provide a lab requisition form that you can use after your visit. Please help us remember to provide you with a form for your next follow-up visit. If a radiology study needs to be scheduled prior to your next visit, we will ask you to contact us at least two to three weeks prior to your next visit so that we will have sufficient time to schedule the examination and obtain approval from your insurance company. [Top] Test results: In order to prevent interruption to patient care, please do not contact our office for lab or x-ray results prior to your appointment. One of our practitioners will review the results with you at time of your appointment. If labs or x-ray tests were not performed prior to your office visit, you will be called with the results within a week or so after the study was performed. Confidentiality: In order to minimize confusion and preserve confidentiality, we prefer that you personally speak with our staff to schedule your own office appointments, diagnostic tests, and surgical procedures (unless there are extenuating circumstances). Insurance policy restrictions: Please inform our staff if your insurance plan requires utilization of a particular hospital, lab or radiology clinic. If you do not know, please contact the member services department. The appropriate phone number is listed on the back of your insurance card. [Top] Changes since your last visit: Please inform our staff if you have any new medical problems, medications, allergies, or changes in your address or health insurance since your last visit. Prescription refills: Please bring a list of your medications with you to your office visit. If we prescribe long-term medication, and you need a new prescription, please remind us to renew your prescription(s) at the time of your visit. If you forget to remind us at the time of your visit, or if you need a refill prior to your next scheduled appointment, please contact your pharmacy and ask the pharmacist to fax a prescription renewal form to our office. Allow at least two weeks prior to the prescription expiration date. In the interest of good medicine, we will only renew prescription medication for patients that have been reevaluated by one of our providers at least once a year. Phone messages and return phone calls: Please organize your thoughts prior to calling our office. If you leave a message in our voice mail system, please clearly state your name (and spell it, if it will be helpful) and contact information at the beginning and at the end of the message. Please be concise and state the reason for your phone call. Phone calls will not be returned if the reason for the phone call is not stated. [Top]
